Mad Rat is the titular protagonist of Mad Rat Dead. He is a lab rat on the verge of death, granted a chance to relive his final day in order to fulfil his life's dream and die happily -- in his case, taking revenge on his human scientist owner for giving him such a dismal existence.

The Mob Rats are NPCs turned special enemies in Mad Rat Dead, first appearing in Stage 1-4 ('Caged Friends'). They are later a minor Boss in Stage 1-5 ('VS Mob Rat'). By the end of the game the mob rats are very likely still alive due to mad rat's intervention.
